IN ORDER TO REQUEST THE DAYS YOU WISH TO ENROLL THIS FORM MUST BE COMPLETED.
THIS DOES NOT GUARANTEE ENROLLMENT IN REQUESTED DAYS. THE HAYDEN AFTER SCHOOL DIRECTOR WILL EMAIL YOU WITH THE STATUS OF YOUR REQUEST

-This program is for children enrolled in LPS Elementary Schools.
-Hayden After School will have a 2 block minimum enrollment requirement.
-The program will not discriminate in providing service to children and their families on the basis of race, religion, cultural heritage, political beliefs, national origin, sexual orientation, disability, or marital status.
